Mission Statement
The CVH is dedicated to the cause of providing veterans and their families with the information and support needed to enjoy their lives with both respect and dignity. It is our organization’s mission to address the social determinates of health (SDoH), educate, coordinate, and provide relief to veterans their families. We strive to ensure that our nation’s veterans obtain the support, benefits, and treatment they have earned from their years of military service and dedication to our country’s sovereignty. We accomplish this goal by providing direct access to critical information related to issues which commonly plague veterans, such as how to obtain safe housing, proper nutrition, relevant medical assistance, and social support.
Our History
Many veterans do not have the resources of background to understand what is going on with their health. Many needs help with housing, winter fuel, and food on the table for their families. Dennis Graham RN NP PhD, an infantry Vietnam Veteran, became a nurse and then a nurse practitioner after and in his daily patient exposure, many were veterans who never spoke of about their military experience. He created CVH to help veterans find their way to get better health care by being more knowledgeable and telling their providers about their military service. The materiel here is based on my knowledge and study of the veterans’ health issues.

In order to provide assistance, CVH will need to validate your veteran status using a copy of your DD-214 or an active VA ID card. We advise having one of these documents on hand prior to contacting us, if possible. If you do not have a copy of either of these documents, please let us know and we can help you obtain one.
Please be aware that CVH will require assistance from your local American Legion or Veterans of Foreign Wars Post in order to carry out our assistance programs.
